The purpose of this Consent for Release (“Consent”) is to enable my participation in the exchange program, and to permit The University of North Carolina at Chapel Hill (“UNC”) to provide information as requested or required by U.S. and foreign governmental agencies. I hereby authorize the Study Abroad Office (“SAO”) at UNC to release information and documents from my education records (as described below) to other units at UNC, my parents or legal guardians, emergency contacts, hospitals and/or healthcare providers, and all appropriate U.S. and foreign governmental agencies: Academic transcripts, letters of recommendation, or other records relating to my academic performance;
Copies of study abroad application materials (including but not limited to course/study plans, copies of my passport, passport photos, housing contracts, medical records);
Records reflecting disciplinary issues, sanctions or proceedings;
Information regarding health, medical or emergency situations which may affect my study abroad program;
Records reflecting financial aid and student accounts affecting my study abroad status;
Other personally identifiable information as deemed necessary by the SAO.
I hereby release, hold harmless, and forever discharge UNC, its trustees, employees, and agents, from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to UNC’s release of my education records and information derived from them to the recipients identified above. This Consent shall remain in effect until revoked. I understand that any revocation of this Consent has only prospective effect and that this Consent cannot be withdrawn retroactively. A copy of this Consent shall have the same force and effect as the original.
In order to be considered for an exchange at the University of North Carolina at Chapel Hill (“UNC”), you are required to read the following information and acknowledge your understanding by ticking the box below. Exchange Student Requirements Listed below are some of the requirements of your exchange program (“Program”). If you are accepted to study at UNC, you will be fully subject to UNC rules and policies, including the Instrument of Student Judicial Governance. You are responsible for:
Checking your email regularly (the email address in your application and your assigned UNC email address) for communication from UNC before, during, and after your Program.
Reading all information and completing all tasks from the Study Abroad Office.
Obtaining a passport and visa in a timely manner.
Submitting all forms or items requested by the Study Abroad Office on time.
Meeting with a representative at your home institution post-acceptance to ensure your courses are approved for credit.
Attending an incoming exchange student orientation.
Paying all fees and personal expenses incurred while in the Program, including fees to UNC and to any other providers you contract with for other services.
Contacting your UNC Exchange Student Coordinator with any questions before, during, and after your Program.
Carrying a full academic course load and completing all academic work during your Program.
Behaving in a manner respectful of the culture and academic standards at UNC.
Exchange students at UNC have access to the following services:
Assistance with the application process, course enrollment, housing, and other queries
An orientation prior to the start of classes that will cover issues and concerns relevant to studying at UNC on exchange
Mandatory medical insurance available through UNC
In an effort to maintain a safe learning community, we must ask the following questions of all applicants. We cannot accept your application unless you answer these questions and provide the required documentation. Your "yes" answer to one or more of the following questions will not necessarily preclude your being admitted. However, your failure to provide complete, accurate, and truthful information will be grounds to deny or withdraw your admission, or to dismiss you after enrollment. For the purpose of the following questions, "crime" or "criminal charge" refers to any crime other than a traffic-related misdemeanor or an infraction. You must, however, include alcohol or drug offenses whether or not they are traffic-related. If you answer "yes" to any of the questions below, you are required to provide your own written explanation of the event and appropriate supporting documentation, i.e., a statement from your legal representative summarizing the event(s) and the final disposition of your case, or a statement from an appropriate school official corroborating your summary of events. If additional information is needed, you may be asked to submit information for a criminal background check at your own expense.

Under the University's system of student self-governance, each student is responsible for obeying and supporting enforcement of the Honor Code. The Honor Code specifically prohibits lying, cheating or stealing and any conduct that impairs significantly the welfare or the educational opportunities of others in the University community.
In our effort to coordinate the logistics of your travel and stay at UNC, we request the following information. The questions below are optional and will not affect your acceptance into the program. Please note, although disclosing the following information is at your discretion, we cannot guarantee personalized requests in your accommodations that are not disclosed to us in advance.
